It is critical that you report your injury to your employer, who will need to open a claim. We will need this claim information in order to schedule your appointment.

Please provide your Workers’ Comp adjuster contact details, so we can obtain authorization and verify the address to submit claims. If don’t have this information, please provide your employer’s contact information.
